Your privacy

ISMP Canada is committed to protecting the privacy, confidentiality and security of all information submitted in error reports. ISMP Canada will not release, publish or keep any information that could identify you or identify a patient, a health professional or an organization involved in a medication error.

ISMP Canada has implemented a number of safeguards to protect information against loss or theft, as well as unauthorized access, disclosure, copying, use or modification. The safeguards include physical measures (for example, locking filing cabinets and restricting access to offices), administrative measures (for example, limiting access to information on a “need-to-know” basis) and technical measures (for example, using passwords and encryption). The processes and practices used to collect, analyze and communicate medication error reports from consumers will be consistent with ISMP Canada’s Privacy Policy.

Please remember that ISMP Canada does not want you to include information in the error report that directly identifies a patient, or that could be combined with other information to identify a patient. Your error report must not include any name, address, telephone or fax number, email address, health care number or any other information that could identify an individual patient. If you are reporting an error that happened to another person, please do not refer to a name or use “he/she”, “son/daughter”, “husband/wife”, “mother/father”, or any other indirect reference that could identify the person who experienced the error. Please try instead to talk about “the patient.” Please do not include the name(s) of health professionals or health care organizations in your report. We need information about the error, not about the people involved.

You will be invited to provide your contact information at the end of the reporting process. Providing contact information is optional. By providing your contact information, you are consenting to be contacted by ISMP Canada for follow-up. This will allow ISMP Canada to contact you if we need clarification or more information about the error. After the error report is fully reviewed and analyzed, ISMP Canada will not keep information that could identify individuals who agreed to be contacted.
